Notice of Data Security Incident
EPIC Health is committed to transparency. The following notice describes a recent data security incident and the steps we have taken in response.
What happened
On January 7, 2026, EPIC Health detected unusual activity within portions of our network. We promptly launched an investigation with leading cybersecurity experts and engaged law enforcement. The investigation determined that an unauthorized actor accessed certain systems on or around January 3, 2026.
Information involved
The information potentially affected may include name, date of birth, address, phone number, email address, patient ID, insurance payor, and diagnosis category. The investigation found no evidence that Social Security numbers or electronic medical records (EMR) were involved.
What we are doing
EPIC Health has taken steps to further strengthen the security of our systems, including enhanced monitoring, additional safeguards, and ongoing employee training. We are providing notice to potentially impacted individuals in accordance with applicable laws.
